After controlling the encounter from the outset, the Bahamas were forced to show resolve late in the last quarter in order to hold off the resurgent Barbadians and come away with a narrow 75-73 win, in the third game of the evening.
Trinidad & Tobago, meanwhile, used a big final quarter to defeat one of the tournament favourites Jamaica, 88-76, while Cuba overcame Antigua & Barbuda, 91-79, and the British Virgin Islands crushed Bermuda, 85-39.
Bahamas got out of the blocks quickly against Barbados, racing to a 27-23, first-quarter lead behind eight points from Alonzo Hinds and seven from Quentin Hall.
At half-time, Barbados found themselves down 43-37 and things grew worse when Bahamas sparked a 9-2 run at the start of the third, to lead 61-50.
Trailing 65-52 at the end of the third, Barbados started strongly in the fourth stanza, outscoring Bahamas 8-2 to reduce the deficit to 67-70.
The Barbadians further chopped that lead to 74-71 following a 7-0 run with 4:29 minutes left on the clock, but then missed crucial baskets in the dying moments to lose out.
For the Bahamas, Hinds scored 15 points while Brian Bain supported with 12.
Barbados were led by Kelvin Patterson with 15 and Sydney Rowe who chipped in with 13.
Jamaica had a disappointing start to their campaign when they went down to T&T, losing concentration late on to capitulate.
They were undermined by Steven Lewis who poured in 27 points to lead T&T, with Reginald George scoring 18 and Wilfred Benjamin, 14.
Jamaica were propelled offensively by Samardo Samuels who finished with 22 points and and Oritseweyinmi Efejuku who had 21.
T&T grabbed the early advantage in the contest, dominating in the paint and from the perimeter to snatch a 43-26 lead at half-time.
Jamaica came out roaring for the second half, embarking on a 17-3 run to take a 52-51 lead before finishing the quarter tied at 58.
The effort seemed to take too much out of the Jamaicans and T&T blew them away 30-18 in the final quarter to win emphatically.
Cuba claimed their win courtesy of a strong offensive showing where Geoffrei Silvestre netted 25 points and Allens Jemmot collected 17 points. For Antigua, Wayne Walker scored 20 points while Kareem Meade got 18.
In the other match played, Kennedy Wellington and Kleon Penn both had 15 points to engineer the BVI's rout of Bermuda.
