Missing docket at KPH is a real pain

Could you kindly investigate this matter as my docket at the Kingston Public Hospital cannot be found?
I have a hernia and have been visiting the KPH. In September 2017 I was referred by the urologist at the surgical clinic and was given an October 2017 date to see a doctor there.
However, when I went to the surgical clinic at the hospital in October to see the doctor, my docket could not be found. I am in excruciating pain and would appreciate your help in this matter.

In October Tell Claudienne contacted the KPH and was told by the Patient Affairs spokesperson that a search was being done for your docket. You were then given an appointment date for early November 2017, but the docket still had not been found.
You were then given a November 24, 2017 appointment date to see a doctor at the surgical clinic, but when Tell Claudienne communicated with the KPH on November 21, 2017, we were told that a search was still being done for the docket.
The Patient Affairs spokesperson said that if the docket was not found by Friday, November 24, 2017, she would speak with the urologist who treated you to find out if a temporary docket could be made.
We note that you were seen by the doctor in the surgical clinic. and that the hernia operation has been done.
