The Greenvale-based outfit triumphed 4-2 on penalties following a 1-1 stalemate in full and extra time.
Ricam had taken the lead in the 26th minute through Kemar Ricketts, only for Jermaine Green to send the game into overtime with a stoppage-time equaliser for Hillstars.
Despite entering the shoot-out with the momentum, Hillstars could not hold their nerves from the spot, missing two of their four penalties, while Ricam converted four from five to claim their first hold on the KO trophy.
Ricam, who will represent Manchester in the all-island KO competition, will now look to add the Major League gong to their collection.
They currently lead Zone Two of that competition on 16 points, following five victories and one draw in their first six games.
